Position: Case Manager - Continuing Care Social Worker

Location: 100% Remote (Living in CA)

Duration: Through EOY 2024

Start Date: ASAP

This role will assist our client in remaining within DHCS compliance by communicating needs and resolving issues with members and local care providers, specifically supporting the Enhanced Care Management CalAIM Initiative.

Responsibilities:

• Assist in implementing program involved with individuals in or recently out of Child Welfare/Foster Youth System
• Interact and coordinate with support staff, including LVNs, RNs, Physicians, Social Workers, etc..
• Member facing activities, communicating between internal and external stakeholders (vendors, hospitals, members, etc..)
• Analyzing member data to improve geographically allocated care

Requirements:

• Masters Degree in Social Work
• Experience in the hospital space within a health plan
• EMR/EHM experience - Epic preferred
• Experience in Medicaid (preferred Medi-Cal) specifically *Managed Care Plans*
• Strong fluency in Microsoft Office Suite - heavy Excel use
